Frozen embryo transfer vs fresh transfer: what the choice actually involves

Should you choose fresh or frozen embryo transfer?
When was first developed, all embryo transfers were 'fresh'. The clinic stimulated your ovaries, collected eggs, fertilised them, and put one back into your uterus a few days later. Freezing was reserved for the leftover embryos, in case the fresh transfer didn't work. As freezing technology improved, clinics started transferring frozen embryos in later cycles even when fresh ones were available, because outcomes looked similar or better in some patient subgroups.

Should I do fresh transfer or freeze all and transfer later?
For most patients, both fresh and frozen elective transfer give broadly similar live birth rates. UK NICE and most international bodies treat them as alternatives.
Where it gets more nuanced
What we honestly do not know
We do not know whether modified-natural-cycle FET, programmed-cycle FET, or natural-cycle FET produces the best outcomes for your specific case. Different clinics have strong preferences with limited head-to-head trial data. We do not know the long-term consequences of artificial-cycle FET on offspring beyond a few years.
Bottom line
If your clinic recommends freeze-all without explaining why for your specific case, ask: am I a high responder? Am I at OHSS risk? Is there an endometrial reason? If the answer is 'this is just our default', it is fair to ask what your individual numbers suggest.
